As the parent or legal guardian of the participant attending the Youth Dance hosted by NSC, I understand that there are certain risks associated with participating in recreational activities, including the potential for injury. I agree to assume full responsibility for any risks, injuries, or damages that may occur to my child as a result of their participation in this event.

By signing below, I hereby waive, release, and discharge NSC, its staff, volunteers, and affiliates from any and all claims for injuries or losses which may arise from participation in this event.

I also grant permission for photos or videos of my child taken during the event to be used on NSC social media platforms, including Facebook, for promotional purposes.

Participant Conduct Agreement

All youth attending this event must read and sign before entering.

In order to make this dance safe and fun for everyone, all participants are expected to follow the rules below. This is a dry event which means no drugs, alcohol, or smoking/vaping of any kind is allowed.

By signing below, I agree to the following:

- I will respect all event staff, volunteers, and other attendees.

- I will keep my behaviour appropriate which means no fighting, bullying, or harassment of any kind.

- I understand that this is a substance free event and I will not bring/use any alcohol, drugs, vapes, or tobacco products at the event.

- I understand that once in the hall, I will not be able to exit unless I am leaving the event.

- I will keep my phone use respectful, meaning I will not film others without consent.

- I understand that all staff and volunteers are here to make sure the event is safe and fun for everyone. I must listen to and follow their directions at all times. If a staff member or volunteer asks me to stop a certain behaviour, I am expected to comply.

- I understand that breaking these rules means I will be removed from the event and my parents/guardian will be contacted.
